Requirements

  • New Community Health Workers (CHWs) will be required to attend the New Jersey Department of Health, Colette Lamothe Galette Institute training and coursework.
  • Candidate must be fluent with proficiency in the following language(s): Spanish and English.
  • Candidate must be a resident of New Jersey.
  • Must have shared life experiences in the communities they serve with shared values to support as well as become an intermediary link between health/social services and the community in order to facilitate access to services and improve the quality of service delivery system.
  • Understand the impact of cultural diversity, cultural humility and cultural responsiveness training.
  • Promote an environment of care, which is inclusive and respectful of every internal and external stakeholders.
  • Effective problem-solving, organizational, and time management skills.
  • Decision-making, priority setting, and work delegation abilities.
  • Administrative/Technological skills, including Microsoft Office (Excel, Word, PowerPoint, Access), and Outlook.
  • Excellent knowledge of available community services and resources.
  • Ability to work independently or in groups, meet strict deadlines, and manage or direct group activities.
  • Ability to attend evening and weekend meetings.
  • Strong communication and interpersonal skills.
  • Comfortable with public speaking and facilitating events in the community.
  • Maintain a high level of confidentiality.
  • Excellent customer-focused interpersonal skills to interact effectively with health care practitioners, the interdisciplinary health care team, community agencies, patients, and families with diverse opinions, values, and cultural ideals.
  • Must have a Valid NJ drivers’ license and own an insured car; position requires some travel within the counties.
  • Must be able to work legally in the United States.
  • High school degree or equivalent combination of education and related experience.
  • Demonstrated experience in community outreach.
  • Proficient in Word, Excel, and Outlook.
  • Ability to work independently and in a team environment, prioritize work, manage multiple tasks, meet deadlines, and quickly adjust or change course as needed.
  • Excellent interpersonal skills, communication skills, flexibility, and attention to detail.
  • Demonstrated ability to work with culturally diverse populations.
  • Comfort traveling and working directly at the community level.

Responsibilities

  • Work to identify and create a caseload of individuals in the community who have physical, behavioral and social health needs.
  • Complete at least 50 preventive screenings per month of uninsured and underinsured residents to identify physical health, behavioral health, and health-related social needs.
  • Ensure at least (95%) of all outreach, screening, referral, and follow-up encounters are documented in CommCare application.
  • Conduct follow-up attempts for (100%) of referred participants.
  • Ensure at least 95% of individuals have data fully entered across all required fields in CommCare application.
  • Participate in RHC-SJ training and meetings, as necessary.
  • Distribute and track RHC-SJ health education materials in the target areas, as appropriate.
  • Conduct outreach to residents and community groups.
  • Develop and maintain relationships with community partners and organizations to expand resources available to community members.
  • Attend or conduct community meetings and events, aiding and advocating where necessary.
  • Maintain accurate documentation of all activities for accountability purposes.
  • Attend vigils for shooting victims in the assigned area to support victims and community members and increase program visibility.
  • Addressing outreach needs in a timely and effective manner.
