About The Position

The RN Complex Care Manager engages designated patient populations to provide complex care management and chronic disease management and achieve optimal outcomes by assessing, planning, implementing, coordinating, and monitoring interventions. The role develops a comprehensive plan of care with patients, patient designees, providers, and the care team to set and monitor goals. It evaluates and coordinates options and services across the continuum of care, collaborating with patients to address complex medical and clinical needs associated with illness, injury, or chronic disease. The manager demonstrates the knowledge and skills needed to serve the assigned patient population and works with Care Management Division leadership to align with strategic goals and OSF Key Results. This position covers the Central Western Region pediatric population, including Galesburg, Monmouth, and Kewanee, with potential travel for patient needs. Work from home one day per week is available after successful completion of the 90-day orientation.

Requirements

Bachelor's degree in Nursing must be obtained within three years of hire into the role.
Three years of experience caring for patients.
Care management experience with adults and/or pediatric patients specializing in chronic, complex illnesses.
Broad knowledge of medical and community resources.
Positive, empathetic approach and ability to communicate effectively with patients and families across health issues, age groups, and socioeconomic backgrounds.
Current state RN license or license eligibility at time of hire.

Nice To Haves

Bachelor's degree in Nursing.
Three years of nursing experience.
Care Management Certification within three years of hire.

Responsibilities
