About The Position

Requirements

• Graduate of an accredited nursing program.
• At least 3 years of related clinical experience in an acute care setting; emergency department or critical care experience is preferred.
• RN license in the states where the applicable HCA facilities are located; a multistate license is required if the transfer center covers more than one state and may be obtained upon hire.
• Knowledge of hospital and clinical operations and patient care practices.
• Problem-solving, analytical and critical-thinking, prioritization, negotiation, conflict-resolution, and proactive decision-making skills.
• Ability to establish and maintain collaborative, effective working relationships.
• Effective oral, written, and electronic communication skills.
• Ability to manage multiple tasks simultaneously.
• Ability to obtain knowledge of state and national rules and regulations, including EMTALA.
• Independent, driven, self-motivated approach, with strong work ethic, integrity, and oral and written presentation skills.
• Proficiency in Microsoft Office applications, internet searches, and relevant databases.
• Ability to work independently and under pressure and follow through on issues.
• Ability to work effectively with employees and external parties.
• Ability to work 12-hour shifts, with scheduling flexibility and rotating weekend and holiday shifts.

Responsibilities

• Perform transfer center functions, including centralized patient placement, promptly and professionally using facility service profiles, clinical protocols, patient care protocols, and healthcare information.
• Coordinate emergent, urgent, and elective patient transfers from referral sources such as hospitals, clinics, post-acute centers, and physician offices, facilitating acceptance and timely decision-making.
• Facilitate patient throughput to the destination facility, focusing on quality and efficiency and following up with transfer participants.
• Promote teamwork and collaborate with colleagues.
• Ensure timely and appropriate bed placement and assignment, and provide accurate patient information to support an initial plan of care.
• Collect and maintain accurate, concurrent information in patient records and center workflow systems.
• Navigate and document in technology platforms, call systems, and office-oriented systems.
• Use clinical judgment, critical thinking, and acquired knowledge to assess patient acuity and facilitate patient flow through the continuum of care.
• Triage and prioritize patient information and communicate accurate clinical presentations to accepting physicians, nurses, and hospital administrators.
• Apply knowledge of human growth and development, assessment, treatment, and care for patients of all ages and clinical service needs.
• Serve as a consultant and mediator for physicians and their offices, nurse practitioners, physician assistants, nurses, and ancillary staff.
• Communicate professionally with referring and receiving facilities and physicians regarding transfers, potential bed placements, and patient movement.
• Use problem-solving and critical-thinking skills to support patient safety and well-being while considering physician preferences and the needs of transferring and receiving hospitals.
• Track and trend barriers to patient intake flow, recommend improvements, and develop action plans.
• Participate in daily bed management meetings when performing the CPP function, and communicate facility resources and issues to appropriate areas.
• Maintain awareness of regulatory requirements, including EMTALA, HCA Ethics and Compliance policies, and quality initiatives; monitor compliance and implement process changes as needed.
• Recommend process changes to improve efficiency and customer experience.
• Use the experience and expertise of other team members when appropriate to determine the best plan of care.
• Work 12-hour shifts, with scheduling flexibility, rotating weekends, and holiday shifts required.
