About The Position

The Registered Nurse provides clinical oversight and support for hospital patient throughput, inpatient discharge, and post-discharge outreach. The role monitors patients for discharge readiness and appropriateness to wait in the Discharge Hospitality Center, completes discharge documentation in the Electronic Medical Record, removes IV and telemetry equipment, and educates patients on inpatient units. The nurse collaborates with case managers, physicians, direct-care nurses, and other care providers to support patient safety and understanding of discharge instructions and timing, and proactively identifies barriers to throughput and discharge. The position also conducts post-discharge outreach with UC Health patients, provides guidance on patient questions and health needs, and follows up on concerns identified during outreach. It coordinates patient flow and discharge priorities with care teams, House Supervisors, and Capacity Management; reviews upcoming admissions and procedure volumes; analyzes Epic data; and contributes to protocols and performance improvement efforts related to capacity, care progression, discharge planning, and patient satisfaction. The nurse uses the nursing process and medical-record review to support clinical care, safe discharge, throughput, and post-discharge follow-up. The role promotes teamwork and patient-centered care, adapts communication to patients’ needs, and responds appropriately to behavioral health concerns and cultural or religious needs.
