About The Position

The Registered Nurse provides clinical oversight and support for hospital patient throughput and the inpatient discharge process. The role monitors discharge readiness and the appropriateness of waiting for transportation in the Discharge Hospitality Center, completes discharge paperwork in the Electronic Medical Record, removes medical devices such as IVs and telemetry equipment, and provides discharge education. The nurse collaborates with case managers, physicians, direct care nurses, and other care providers to promote patient safety and understanding of discharge instructions and timing, while proactively identifying barriers to throughput and discharge. The position also conducts post-discharge outreach to UC Health patients, responds to patient and family questions and health needs, and follows up on concerns to ensure appropriate actions are taken. It coordinates care progression and discharge priorities with hospital teams, analyzes throughput data, supports capacity planning and performance improvement, and promotes patient-centered teamwork and professional leadership.
