About The Position

The Clinical Consultant for the Enhanced Care Management (ECM) - Community Supports (CS) program supports the program and Advocates by assuring the quality of clinical service is being maintained. Ensures that clinical records are accurate, current, and meet Medical Assistance standards and regulations. The Clinical Consultant provides expertise and guidance to improve student care, program effectiveness, and clinical outcomes. Strong clinical background, experience in care management, and a passion for transforming service delivery through innovative practices. ECM is a comprehensive, patient-centered approach to healthcare that aims to improve outcomes for high-need populations. This involves the coordination of medical, behavioral, and social services to address the full range of student needs, focusing on those with complex and chronic conditions. ECM programs are designed to optimize care delivery, reduce unnecessary hospitalizations, and improve the overall quality of life for students.

Requirements

  • Master’s or Doctoral degree in behavioral health, public health, or related human services field
  • Active, unrestricted licensure in California: LCSW, LMFT, LPCC, PsyD, or PhD
  • Minimum 5 years of clinical experience, including: Behavioral health (mental health and/or substance use)
  • Minimum 5 years of clinical experience, including: High-acuity or underserved populations (e.g., homeless, justice-involved)
  • At least 2 years of experience in care management, population health, or ECM-related services
  • Strong experience in: Clinical documentation review
  • Strong experience in: Treatment planning
  • Strong experience in: Crisis intervention
  • Valid and unexpired driver’s license
  • Required to have car insurance, a vehicle, and a valid driver’s license
  • Excellent interpersonal, communication, and facilitation skills, with the ability to effectively engage diverse stakeholders across community and organizational settings
  • Expert knowledge of clinical practices, care coordination, and patient-centered care
  • Strong analytical skills with the ability to interpret clinical and service utilization data and translate findings into actionable strategies
  • Advanced understanding of medical necessity criteria, ECM/CalAIM requirements, behavioral health systems, and regulatory frameworks
  • Strong clinical judgment, assessment, and problem-solving abilities
  • Knowledge of trauma-informed care, reentry services, and working with justice-involved and high-acuity populations
  • Ability to work independently while supporting multiple programs, contracts, and interdisciplinary teams
  • Strong collaboration skills across counties, providers, and multidisciplinary teams
  • Cultural competency and ability to work effectively with diverse and underserved populations
  • Knowledge of contract parameters, deliverables, and compliance with local, state, and federal regulations
  • Ability to support quality assurance efforts, including identifying gaps, recommending improvements, and ensuring program compliance
  • Ability to manage multiple priorities, adapt to changing demands, and implement effective solutions
  • Ability to clearly communicate concepts, prepare reports, and develop policies and procedures
  • Proficiency in Electronic Health Records (EHR) systems and Microsoft Office Suite (Word, Excel, Outlook, etc.)
  • Understanding and ability to apply organizational policies, procedures, and compliance standards
  • Faculty must frequently lift and/or move up to 50 pounds.
  • Frequently required to sit and/or stand for extended periods of time.
  • Frequently required to type for extended periods of time.
  • Occasionally required to reach, walk, climb, pull, push, grasp, stretch, stoop.
  • Frequently required to talk, hear, see, and use repetitive motions.
  • Frequently able to communicate in verbal and written form.
  • Frequently able to use all types of vision, such as close vision, distance vision, color vision, peripheral vision, and depth perception, to complete daily tasks.

Nice To Haves

  • Previous Clinical experience working with individuals recently released from jail or prison a plus.
  • Previous Supervisory experience.
  • Experience providing services inside correctional facilities
  • Knowledge of reentry systems, benefits navigation, and housing resources
  • Experience with ECM/CalAIM programs and Managed Care Plans
  • Experience working with justice-involved or reentry populations
  • Previous supervisory or clinical oversight experience
  • Knowledge of housing resources, benefits navigation, and community-based systems
  • Comfortable working in both secure custodial environments and community-based field settings
  • Bilingual, fluent Spanish/English oral and written skills are highly desirable.
  • Community Health Worker certification through California’s HCAI Department.

Responsibilities

  • Review, approve, and provide clinical sign-off on comprehensive assessments, individualized care plans, and treatment-related documentation
  • Ensure documentation meets medical necessity and ECM/CalAIM standards
  • Provide feedback to staff to improve clinical quality, accuracy, and compliance
  • Maintain consistency in clinical practices across multiple contracts, counties, and Managed Care Plans
  • Serve as a clinical resource and consultant to ECM staff and multidisciplinary teams
  • Provide guidance on complex and high-risk cases, behavioral health and substance use concerns, crisis intervention and stabilization, and treatment planning and care coordination strategies
  • Provide consultation and support to staff, including availability for case debriefing and clinical guidance
  • Assist with cases requiring escalated clinical involvement
  • Provide consultation on both clinical and administrative issues as requested
  • Assist in the development and implementation of clinical strategies aligned with program goals
  • Support the integration of evidence-based practices into ECM service delivery
  • Evaluate and recommend clinical models, care pathways, and best practices to enhance outcomes
  • Collaborate with Program Director and leadership to support program design and clinical effectiveness
  • Support the implementation of Teaching and Therapeutic Community models within programming
  • Provide oversight to ensure ECM services meet local, state, and federal regulations, Managed Care Plan requirements, and contractual expectations and deliverables
  • Conduct chart reviews, audits, and quality checks
  • Identify gaps, trends, and risks in clinical service delivery and recommend improvements
  • Support audit readiness, site visits, and compliance reviews
  • Develop and deliver training on clinical documentation standards, care coordination techniques, student engagement strategies, and best practices in ECM service delivery
  • Support onboarding and ongoing staff development and clinical competency building
  • Promote consistency in clinical approaches across programs and counties
  • Serve as a clinical liaison between ECM programs, healthcare providers, and community partners
  • Collaborate with multidisciplinary teams to coordinate care and improve service delivery
  • Participate in committees and meetings to advocate for ECM services and the populations served
  • Support quality improvement initiatives in partnership with leadership and care teams
  • Provide clinical supervision and guidance to ECM Advocates and Housing Navigators (as required for clinical oversight)
  • Offer ongoing consultation, mentorship, and support without direct managerial responsibility
  • Support staff in applying trauma-informed care and client-centered approaches
  • Attend meetings, trainings, and workshops as requested
  • Maintain documentation of clinical reviews and approvals
  • Provide reports on clinical trends, outcomes, and recommendations
  • Perform other duties as assigned
