About The Position
The Case Manager provides comprehensive case management services to adults experiencing homelessness in a residential hospice setting in Philadelphia County. The position focuses on helping clients achieve housing stability, connect with community resources, and support their overall health and well-being. This is a temporary, full-time position. Occasional evening and weekend hours may be required.
Requirements
- Bachelor of Social Work (BSW).
- A minimum of two years of social work or case management experience.
- Strong computer skills.
- Strong documentation skills.
- Strong communication skills.

- Experience working with individuals experiencing homelessness, mental illness, and substance use disorders.
Responsibilities
- Conduct client intakes and assessments.
- Develop individualized service plans.
- Monitor client progress.
- Assist clients with housing applications.
- Provide supportive counseling.
- Maintain accurate electronic case records.
- Facilitate resident groups.
- Coordinate services with community providers.
- Compile program data.
- Participate in team meetings and required training.
