About The Position
The Licensed Practical Nurse prepares patients for providers and assists with examinations, suture preparation, and provider orders, including supplies, tests, X-rays, and medication dispensing. The role conducts health assessments, prepares patients for physicals, calibrates equipment, and performs clinical testing such as podiatry-related testing, respiratory approvals, spirometry, audiograms, EKGs, drug testing, Timms testing, breath and saliva alcohol testing, and venipuncture. The LPN reviews physical and test results with providers and handles phone calls related to case management and results. Under the direction of the Nurse Practitioner and attending physicians, the LPN implements and evaluates individualized care plans, educates patients on self-management and disease-management skills, and supports coordinated care during and between clinic visits. The role coordinates pre-visit and post-visit planning and scheduling, assists with group visits, helps improve training materials and patient communications, participates in team case conferences, and maintains licensure. Other duties and responsibilities may be assigned as directed.
Requirements
- Degree from an LPN nursing program.
- Current New Jersey LPN license.
- BLS certification required.
- Excellent organizational and interpersonal skills.
- Strong oral and written communication skills.
- Ability to work effectively with people of varied backgrounds, ethnicities, and life experiences.
- Ability to collaborate with a team, manage multiple priorities, use effective time management, and exercise sound administrative and clinical judgment.
- Ability to act independently.
- Ability to work in multiple computer systems.
Nice To Haves
- One to three years of experience providing clinical services in an outpatient setting is preferred; new graduates will also be considered.
Responsibilities

- Implement and evaluate individualized care plans under the direction of the Nurse Practitioner and attending physicians.
- Educate patients on appropriate self-management behaviors and disease-management skills.
- Support coordinated care and patient health outcomes during and between clinic visits.
- Coordinate pre-visit and post-visit planning and scheduling to promote consistent patient engagement.
- Assist in organizing and conducting group visits.
- Assist in optimizing training materials, patient communications, and other documents for program managers.
- Participate in team case conferences to address patient progress and barriers to care.
- Maintain licensure.
- Perform other duties and responsibilities as directed.
