The Nurse Practitioner works across skilled nursing facility and Home Health AI Care programs in St. Joseph, Michigan. The role combines direct patient care with use of predictive health signals, staff coaching, and integration of Circadia technology into clinical workflows. The NP sees new admits, facility-flagged patients, and patients identified through Circadia alerts, and may provide care in patients’ homes.

This is a per-diem role with a schedule of approximately 4–5 visits per month as stated in the role details; the compensation section also describes a minimum of 4–8 days per month, with additional days possible based on patient demand. The position covers designated facilities and patient homes within a 30–40-mile radius. For locations outside that radius, mileage and lodging are covered according to company policy. Compensation is $3,000–$5,200 per month, with a paid-per-visit model.

Requirements

• Certified and licensed Nurse Practitioner in Michigan.
• ANCC certification or equivalent in Adult, Family, Geriatric, or Acute care.
• Eligible for Medicare participation; the company bills on the NP’s behalf.
• At least 2 years of Nurse Practitioner experience with complex adult populations.
• Ability to practice independently.
• Valid Michigan driver’s license and comfort with local travel.
• Ability to thrive in a dynamic, evolving startup environment.
• English proficiency required.

Nice To Haves

• Experience with Google Suite, multiple electronic medical records, Slack, and AI transcription or other AI tools.
• Spanish language skills.

Responsibilities

• Conduct initial and follow-up assessments for flagged residents.
• Initiate or adjust plans of care, complete consent, and coordinate with facility providers.
• Train and coach staff at the point of care and standardize Circadia use in clinical workflows.
• Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Complete documentation and charts on the same day; use AI-powered tools to complete notes within 24 hours of each encounter.
• Efficiently round across designated skilled nursing facilities, typically completing 25 or more visits per day where appropriate, focusing on new admits, facility-flagged patients, and Circadia alerts.
• Provide quality and product feedback by surfacing field insights to improve workflows and technology integration in clinical settings.
• Support Nurse Practitioner onboarding, share best practices, and collaborate with clinical operations and product teams.
• Cover skilled nursing facility sites and patient homes within a 30–40-mile radius.
