About The Position
Circadia Health is a growth-stage healthcare AI company focused on preventing hospitalizations and transforming senior care. The Circadia Intelligence Platform integrates contactless sensing, predictive models, and enterprise integrations to detect adverse events early and operationalize interventions. The company's technology currently serves over 40,000 post-acute patients daily across skilled-nursing, home-health, and home-care networks. This role involves working across skilled nursing and Home Health AI Care programs, providing direct patient care, acting on real-time predictive health signals, and coaching facility teams to utilize data for earlier interventions. The Nurse Practitioner will round on new admits, flagged patients, and Circadia alerts in SNF partner facilities, and extend proactive care into patients' homes in the home health setting. The role blends direct patient care with technology integration through training, demos, and assessments. The position is located in St. Joseph, MI, and is on a per diem basis, requiring 4-5 visits per month.
Requirements
- Certified and licensed Nurse Practitioner in MI.
- ANCC (or equivalent) certification in Adult/Family/Geriatric/Acute.
- Eligible for Medicare participation.
- 2+ years of NP experience with complex adult populations.
- Ability to practice independently.
- Valid MI driver's license.
- Comfortable traveling locally.
- Ability to thrive in a dynamic, evolving startup environment.
- English proficiency required.
Nice To Haves
- Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
- Spanish language proficiency is preferred.
Responsibilities
- Conduct initial and follow-up assessments on flagged residents.
- Initiate/adjust plans of care.
- Complete consent forms.
- Coordinate with facility providers.
- Train and coach staff at the point of care to standardize the use of Circadia into clinical workflows.
- Interpret trend-based insights from data.
- Prioritize high-risk patients.
- Drive timely interventions to prevent patient decline.
- Complete documentation and charts within the same day.
- Efficiently complete visits across designated SNFs, focusing on new admits, facility-flagged patients, and Circadia alerts.
- Surface field insights to improve workflows and tech integration in a clinical setting.
- Support NP onboarding.
- Share best practices.
- Partner with clinical ops and product teams.
- Cover SNF sites and patient homes within a 30-40 mile radius.
- Travel to locations outside the radius, with mileage and lodging covered per company policy.
