About The Position

VNAcare is seeking a Nurse Practitioner to provide professional and comprehensive care to critically, chronically, or terminally ill patients in the community. The position is based in Rancho Cucamonga and surrounding areas and involves working within an interdisciplinary setting. The role includes scheduling home visits for patients who need face-to-face visits for recertification. This is a per diem position on the day shift. VNAcare describes its clinicians’ schedules as flexible, with limited weekends, after-hours, and on-call work, and offers orientation and on-the-job training.

Requirements

Current Registered Nursing license in the State of California.
Current Nurse Practitioner license in the State of California.
At least one year of professional nursing experience.
Ability to work within an interdisciplinary setting.
Understanding of hospice philosophy and the principles of death and dying.
Ability to comply with accepted professional standards and practices.
Excellent observation, assessment, nursing judgment, and communication skills.
Ability to provide accurate written documentation in a timely manner.
Self-directed ability to work with little supervision.
Ability to efficiently schedule home visits for patients needing a face-to-face visit for recertification.

Nice To Haves

Hospice experience preferred.
