As a home care registered nurse, you provide comfort-focused hospice care to patients with prognoses of six months or less and build compassionate, meaningful connections with patients and the people in their lives. You work as part of an interdisciplinary hospice team and advocate for patients and families so their care goals are communicated and prioritized. You also provide basic hospice education and training to patients, families, and caregivers. Most of your time is spent caring for patients wherever they call home, including private residences and assisted living or skilled nursing facilities. You typically make 4–6 visits each day and visit the local program office once a week to meet with an interdisciplinary team that collaborates to support patients and families. The role is grounded in delivering compassionate, personalized care with empathy and honoring each patient’s wishes.
