About The Position

This position involves participating in the delivery of care as set by the care plan created by the RN Case Manager and the interdisciplinary team. The role assists with planning and implementing an ongoing audit reimbursement program in accordance with federal, state, and Hospice standards, under the supervision of a registered nurse. The Licensed Practical Nurse (LPN) will be responsible for making visits as scheduled, maintaining patient rights and dignity, and assuring the care of the dying patient, optimizing their comfort and dignity. This role requires flexibility and agility as a field LPN, reporting findings and concerns to the RN Case Manager, and auditing one complete chart for each field nurse in the QAPI team monthly. The position requires adequate knowledge of Medicare/Medicaid hospice benefit and insurance standards to ensure reimbursement for all services rendered and the regulatory standard of care is provided.

Requirements

  • Successful completion of LPN or LVN training program
  • 1 year related experience
  • Current LPN license
  • driver’s license and current automobile liability insurance

Nice To Haves

  • Previous hospice or home health experience a plus
