LVN / LPN – Behavioral Health
About The Position
Tryfacta is seeking an LVN / LPN – Behavioral Health for a client in Newark, New York, for a 13-week contract assignment. The position provides nursing care to residents in a secured behavioral health unit, including medication administration, monitoring and documenting residents’ conditions, behavioral management, and support with daily living and personal care. The nurse collaborates with healthcare professionals, maintains accurate documentation in EPIC, supports discharge planning, and follows infection control, safety, and confidentiality protocols. The schedule is mid shift, five 8-hour shifts from 14:00 to 22:00. Pay is $30.00–$43.00 per hour.
Requirements
- Active New York LPN license in hand at the time of submission.
- Minimum 2 years of nursing experience.
- EPIC experience is mandatory.
- Current BLS certification.
- Ability to manage challenging behaviors and follow de-escalation protocols.
- Availability to work every other weekend.

- Previous behavioral health, dementia care, or long-term care experience.
Responsibilities
- Provide nursing care to residents in accordance with established care plans and facility policies.
- Administer medications and monitor residents’ responses to treatment.
- Monitor and document vital signs, intake and output, and changes in residents’ conditions.
- Implement behavioral management and de-escalation techniques.
- Assist with managing residents who may exhibit verbal or physical aggression.
- Maintain patient safety and follow protocols for a secured behavioral health unit.
- Collaborate with RNs, physicians, and other healthcare professionals to support resident care.
- Support residents with activities of daily living and personal care as needed.
- Maintain accurate documentation using EPIC.
- Assist with discharge planning and help residents transition to the least restrictive appropriate living environment.
- Follow infection control procedures and maintain patient confidentiality.
- Float based on facility needs.
