About The Position
The Licensed Practical Nurse (LPN) administers nursing care to residents under the direction of a Registered Professional Nurse (RN). The LPN assists with treatments and procedures and carries out selected duties within the established nursing plan of care. This is a full-time evening position at Heritage House, a skilled nursing facility providing long-term and subacute care in Troy, New York. The scheduled shift is 3 p.m. to 11 p.m. The LPN works with the RN and other healthcare team members to support safe, quality, person-centered resident care. Requirements
- Current license to practice as a Licensed Practical Nurse (LPN) in the State of New York.
Responsibilities
- Collects resident data in collaboration with the RN and determines care priorities based on identified needs and goals.
- Contributes to the plan of care through collaboration and effective communication with the RN and other healthcare team members.
- Provides safe, quality resident care in accordance with established nursing policies, procedures, standards, and the scope of practice, under the direction of the RN.
- Administers medications and monitors IV therapy as ordered and according to established policies and standards.
- Assists with treatments and procedures and carries out selected duties within the established nursing plan of care.
