About The Position

Heritage House is seeking a dedicated, compassionate Registered Nurse to join its part-time day team in sub-acute rehabilitation and skilled nursing. The RN uses the nursing process to diagnose and treat human responses to actual or potential problems of individuals or groups. Heritage House is an Eddy facility in Troy, New York, providing long-term and sub-acute care since 1992. Requirements

Current license to practice as a Registered Nurse in the State of New York.
Associate’s degree in nursing or a nursing degree from an accredited school.
Passion for caring for patients and a respectful, caring, professional demeanor.

Nice To Haves

One or more years of experience as an RN in a skilled nursing setting.
Bachelor’s degree in nursing.

Responsibilities

Complete admission assessments in accordance with guidelines and interview residents and significant others to obtain health histories.
Use available resources to assess residents with conditions that are new, unfamiliar, or not commonly seen on the unit.
Interpret overt and covert assessment data to identify when a resident is at acute psychological or psychosocial risk, and report and document the interpretation.
Inform and educate residents about the care or services to be provided.
Develop and contribute to plans of care based on potential existing problems, expected resident responses, and the medical plan of care.
Communicate plans of care using methods that include action plans, goals, and time frames.
