About The Position

The Licensed Practical Nurse (LPN) provides nursing care to residents in a skilled nursing setting under the direction of a Registered Professional Nurse. The role assists with treatments and procedures and performs selected duties within the established nursing plan of care. This is a full-time evening position scheduled from 3:00 p.m. to 11:00 p.m. at Eddy Heritage House, a long-term and subacute care facility in Troy, New York.

Requirements

• Current license to practice as a Licensed Practical Nurse (LPN) in the State of New York.

Responsibilities

• In collaboration with the RN, collect resident data and determine care priorities based on identified needs and goals.
• Contribute to residents’ plans of care through collaboration and effective communication with the RN and other healthcare team members.
• Follow established nursing policies, procedures, and standards under the RN’s direction and within the scope of practice to provide safe, quality care.
• Administer medications and monitor IV therapy as ordered and according to established policies and standards.
• Assist with treatments and procedures and carry out selected duties within the established nursing plan of care.
