About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse to provide one-on-one pediatric private duty nursing care for a 13-year-old child in a supportive home setting in Monessen, Pennsylvania. The role focuses on personalized, family-centered care and building a meaningful relationship with the child and family.

This is a full-time day-shift position, Monday through Friday, for eight hours per day. The nurse will provide daily care and monitoring, perform g-tube care and feedings, and communicate with the care team and family.

Requirements

• Current LPN or RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience.
• CPR certification.
• Reliable transportation and a valid driver's license.
• A caring attitude, strong independence, and a passion for pediatric care.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Assist with activities of daily living (ADLs).
• Perform g-tube care and feedings.
• Monitor seizures and vital signs.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
• Deliver exceptional, family-centered pediatric care.
