About The Position
Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse to provide one-on-one pediatric private duty nursing for a 20-year-old male in a supportive home setting in Portage, Pennsylvania. The role focuses on personalized, family-centered care and building a meaningful connection with the patient and family. This is a full-time, Monday-through-Friday day-shift position scheduled from 6:30 AM to 3:30 PM. The nurse will provide assistance with activities of daily living, g-tube care and feedings, vital-sign monitoring, seizure monitoring, and safe medication administration, while communicating with the care team and family.
Requirements
- Current LPN or RN license in Pennsylvania.
- Graduate of an accredited nursing program.
- At least 6 months of nursing experience.
- CPR certification.
- Reliable transportation and a valid driver's license.
- Compassion, strong independence, and a passion for pediatric care.
Nice To Haves
- Home health experience is preferred, but not required.
Responsibilities
- Assist with activities of daily living (ADLs).
- Provide g-tube care and feedings.
- Monitor vital signs.
- Monitor seizures.
- Administer medications safely and accurately.
- Communicate clearly and compassionately with the care team and family.
