LPN | Full Time Daylight | Portage
About The Position
Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide one-on-one pediatric private duty nursing care for a 20-year-old male in a supportive home setting in Portage, Pennsylvania. The role focuses on personalized, family-centered care and building a meaningful connection with the individual and family. This is a full-time day-shift position, Monday through Friday, from 6:30 AM to 3:30 PM. The nurse will provide daily living assistance, g-tube care and feedings, vital and seizure monitoring, and medication administration while communicating with the care team and family.
Requirements
- Current LPN or RN license in Pennsylvania.
- Graduate of an accredited nursing program.
- At least 6 months of nursing experience; home health experience is preferred but not required.
- CPR certification.
- Reliable transportation and a valid driver's license.
- A caring disposition, strong independence, and a passion for pediatric care.

Responsibilities
- Assist with activities of daily living (ADLs).
- Perform g-tube care and feedings.
