About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse to provide one-on-one pediatric private duty nursing for a 3-year-old child in a supportive home setting in Portage, Pennsylvania. The role is part-time, with a day-shift schedule of three days per week from 8:00 AM to 5:00 PM. The nurse will provide personalized, family-centered care and work closely with the child’s family and care team.

Requirements

• Current LPN or RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience; home health experience is preferred but not required.
• CPR certification.
• Reliable transportation and a valid driver’s license.
• A caring attitude, strong independence, and a passion for pediatric care.

Responsibilities

• Assist with activities of daily living (ADLs).
• Perform G-tube care and feedings.
• Monitor vital signs.
• Provide tracheostomy and ventilator care and management.
• Perform respiratory assessments.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
• Deliver exceptional, family-centered pediatric care.
