About The Position

Community Care, Inc. is seeking a compassionate and dedicated LPN to provide one-on-one pediatric private duty nursing care for a 2-year-old male child with cardiac needs in a supportive home setting in Monessen, Pennsylvania. The role focuses on personalized, family-centered care and building a meaningful connection with the child and family.

This is a full-time overnight position scheduled Monday through Friday, 10:00 PM to 8:00 PM, with weekends available. The nurse will provide daily-living assistance, g/GJ-tube care and feedings, aspiration precautions, vital-sign monitoring, medication administration, and communication with the care team and family.

Requirements

• Current LPN or RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience.
• CPR certification.
• Reliable transportation and a valid driver's license.
• A caring heart, strong independence, and a passion for pediatric care.

Nice To Haves

• Home health experience is preferred but not required.

Responsibilities

• Assist with activities of daily living (ADLs).
• Perform g/GJ-tube care and feedings and follow aspiration precautions.
• Monitor vital signs.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
• Deliver exceptional, family-centered pediatric care.
