About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse to provide one-on-one pediatric private duty nursing care to a 12-year-old child in a supportive home setting in Washington, Pennsylvania. The role focuses on personalized, family-centered care and building a meaningful connection with the child and family.

This is a full-time overnight position, scheduled Monday through Friday from 10:00 PM to 6:00 AM. Weekend work is also available. The nurse will provide assistance with daily activities, feeding-tube care, tracheostomy and ventilator monitoring, seizure and vital-sign monitoring, and medication administration, while communicating with the care team and family.

Requirements

• Current LPN or RN license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience; home health experience is preferred but not required.
• CPR certification.
• Reliable transportation and a valid driver's license.
• Caring disposition, strong independence, and a passion for pediatric care.

Responsibilities

• Assist with activities of daily living (ADLs).
• Perform G-tube care and feedings.
• Provide tracheostomy and ventilator care and monitoring.
