About The Position
Community Care, Inc. is hiring a full-time Licensed Practical Nurse to provide one-on-one pediatric private duty nursing care for a 12-year-old child in a supportive home setting in Washington, Pennsylvania. The position is scheduled Monday through Friday from 10:00 PM to 6:00 AM, with weekends available. The nurse will provide personalized, family-centered care, including support for a child with tracheostomy and ventilator needs, and will communicate with the care team and family.
Requirements
- Current LPN or RN license in the state of Pennsylvania.
- Graduate of an accredited nursing program.
- At least 6 months of nursing experience; home health experience is preferred but not required.
- CPR certification.
- Reliable transportation and a valid driver's license.
- A caring disposition, strong independence, and a passion for pediatric care.

Responsibilities
- Assist with activities of daily living (ADLs).
- Provide G-tube care and feedings.
- Provide tracheostomy and ventilator care and monitoring.
- Monitor vital signs.
- Monitor seizures.
- Administer medications safely and accurately.
- Communicate clearly and compassionately with the care team and family.
- Deliver exceptional, family-centered pediatric care.
