About The Position
Community Care, Inc. is hiring an LPN to provide one-on-one pediatric private duty nursing care for a 13-year-old child in a supportive home setting in Monessen, Pennsylvania. The position is full-time, Monday through Friday, with eight hours per day on the day shift. The nurse will provide personalized, family-centered care and work closely with the child’s family and care team. Community Care is a family-owned home health agency that has served local communities since 1987. Its services include skilled nursing and therapy visits, as well as adult and pediatric private duty care. Requirements
- Current Pennsylvania LPN or RN license.
- Graduation from an accredited nursing program.
- At least 6 months of nursing experience; home health experience is preferred but not required.
- CPR certification.
- Reliable transportation and a valid driver's license.
- A caring, independent approach and a passion for pediatric care.

Responsibilities
- Assist with activities of daily living (ADLs).
- Perform G-tube care and feedings.
- Monitor seizures and vital signs.
- Administer medications safely and accurately.
- Communicate clearly and compassionately with the care team and family.
