About The Position

Fairview Range is hiring a Behavioral Health Clinician LICSW for our Behavioral Health Department within the Mesaba Clinic in Hibbing, MN. As a member of the primary care team, the Behavioral Health Clinician assists in the team's management of the overall health care needs of the enrolled patient population. The Behavioral Health Clinician's primary functions include assessment, consultation and direct care related to the prevention, identification, treatment and ongoing management of psychosocial/behavioral concerns which impact the overall health of the defined patient population. Requirements

  • Master of Science in Social Work, Psychology, or related field
  • 1-year prior MH experience
  • Current MN license for: Licensed Independent Clinical Social Worker (LICSW), Licensed Psychologist, or Licensed Professional Clinical Counselor (LPCC)

Responsibilities

  • Conducting assessment and evaluation of behavioral health conditions by means of diagnostic and screening tools, clinical interviews and consultation with the medical team.
  • Contributing to the development and implementation of treatment plan in collaboration with the patient and primary care team.
  • Providing consultation to the medical team aimed at the prevention, identification, treatment and management of behavioral health/psychosocial issues and conditions.
  • Functioning as an effective team member as evidenced by working in a collaborative approach with primary care providers.
  • Understanding of integrative health care in terms of the interactive and interdependent expressions of the physical/biological, emotional and psychological dimensions of health.
  • Employing a primary care approach in addressing the behavioral/psychosocial aspects of routine and complex medical conditions.
  • Providing primary behavioral health interventions for identified mental health and substance use conditions.
