About The Position

The Family Services Manager plays a vital role in ensuring compassionate and high-quality care for hospice clients and their families. You'll lead a team of dedicated professionals in Social Work, Spiritual Care, Volunteer Coordination, and Bereavement Support Services, guiding their work and ensuring seamless collaboration within the Hospice Interdisciplinary Team.

Requirements

  • Master's Degree in Social Work (MSW) required
  • Vermont Licensed Clinical Social Worker (LICSW) required/exam eligible and scheduled
  • Minimum of five years of relevant hospice experience, or a combination of education and experience from which comparable knowledge and skills are acquired.
  • At least two years of supervisory experience.
  • Strong understanding of hospice care, including social, spiritual, and bereavement needs of clients and families.
  • Familiarity with the business of hospice.
  • Excellent communication, interpersonal, and organizational skills.
  • Strong leadership, mentorship, and problem-solving abilities.
  • Demonstrated ability to work collaboratively within an interdisciplinary team.

Nice To Haves

  • Certification in Hospice & Palliative Care or other related specialty preferred.

Responsibilities

  • Supervise, mentor, and develop hospice Interdisciplinary Team members, fostering a supportive and high-performing environment.
  • Oversee the leaders responsible for the delivery of high-quality care, ensuring compliance with all relevant regulations and best practices.
  • Contribute to the ongoing development and improvement of hospice services, including identifying and implementing quality improvement initiatives.
  • Partner with clinical, administrative, and medical staff to ensure coordinated and holistic care for clients. Acts as liaison between senior leadership.
  • Represent the hospice program in the community, participating in educational events and building strong relationships with other healthcare providers.
  • Manage team schedules, budgets, and resources effectively.
