Access Services is hiring a Young Adult Mental Health Case Manager, also known as a TIP Facilitator, to join their Transition to Independence Process (TIP) program serving the Lehigh Valley with an office in Bethlehem, PA. The TIP program is an evidence-based model that supports transition-age young adults (16-26 years old) in achieving their future goals. TIP Facilitators empower young adults by helping them develop life skills, connect to resources, and build independence through a strength-based, person-centered, and youth-driven approach. This role involves community-based services, engaging youth in five key domains (Education; Employment & Career; Living Situation; Community Life Functioning; Personal Effectiveness & Wellbeing), building trusting relationships, supporting the creation of personalized 'Futures Plans', advocating in multi-system meetings, collaborating with various stakeholders, and planning group events. Responsibilities also include completing documentation, meeting billable hour requirements, participating in case management reviews, on-call rotations, and team meetings, as well as representing the TIP Program through outreach.
