RN Case Manager- Hospice - Detroit territory
About The Position
St. Croix Hospice is seeking a Registered Nurse Case Manager based at its Livonia, Michigan branch, serving patients in the Detroit area. The position provides individualized, holistic hospice support to patients in their homes. The nurse collaborates with the care team to develop personalized plans of care, leads end-of-life conversations, and manages symptoms with empathy and clinical excellence. The role offers meaningful patient connections and autonomy in practice. It is full time, scheduled from 8:00 a.m. to 5:00 p.m., with on-call expectations and rotating on-call duties. Travel is 100%.
Requirements
- Associate, bachelor's, or master's degree from an accredited nursing program (RN).
- At least 1 year of recent acute care experience.
- Reliable transportation, a valid driver's license, and proof of auto insurance.
Nice To Haves
- Hospice or home health experience preferred.
Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with patients and families, updating them as needs evolve.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on-call duties and ensure seamless care transitions.
- Collaborate with the care team to support patients and families through end-of-life care.
