RN Case Manager Hospice
About The Position
Amedisys is hiring a full-time, day-shift RN Case Manager Hospice serving Atlantic, Cape May, and Cumberland Counties. The role provides hospice nursing care and supports patients at home. The nurse assesses patients, works with the care team to develop and implement individualized plans of care, documents care and progress, and teaches patients, families, and caregivers. The position also includes assigned on-call responsibilities. The annual salary range is $96,000–$100,000. Requirements
- One year of nursing experience.
- Current RN license specific to the state or states where assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain accurate and timely individualized patient care progress records in accordance with care center policies.
- Record pain and symptom management changes and outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with the clinical manager and care team.
- Perform assigned on-call responsibilities and provide on-call services to patients and families.
- Perform other duties as assigned.
