About The Position

Amedisys is hiring an RN Case Manager Hospice to cover the Knoxville area. The role supports hospice patients in the home by assessing their needs, collaborating with the care team on individualized plans of care, and providing education to patients, families, and caregivers. The nurse maintains timely and accurate patient records, communicates regularly with the clinical manager and care team, and provides on-call services as assigned. A $5,000 sign-on bonus is offered. Compensation may vary based on geographic market, job-related knowledge, skills, and experience. Requirements

• One year of nursing experience.
• Current RN license specific to the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately, timely, and according to care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Perform other duties as assigned.
