About The Position
As an RN Case Manager, you’ll coordinate compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The position is with Amedisys, part of the Optum family of businesses, which provides home health, hospice, and community-based services. The role includes assigned on-call responsibilities and services to patients and families.
Requirements
- Current RN license specific to the state or states where assigned to work.
- At least 1 year of nursing experience.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed or document the rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records accurately, promptly, and according to care center policies.
- Record pain and symptom management changes and outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Communicate regularly with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
