RN Case Manager
About The Position
Lighthouse Hospice is seeking a full-time Hospice Registered Nurse Case Manager to provide end-of-life nursing care throughout the community. The role serves patients and families across Mercer, Burlington, Camden, Gloucester, and Salem counties, delivering hands-on care, coordinating services, and guiding patients and families throughout the hospice journey. The RN Case Manager works as part of an interdisciplinary care team to support comfort, dignity, and quality of life. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with shared call duties. The position includes clinical onboarding and is supported by a growth-oriented environment and a comprehensive benefits package.
Requirements
- Current, unencumbered Registered Nurse license.
- At least one year of RN experience in a medical-surgical or acute care setting.
- Computer proficiency, including typing and use of various programs, with experience in electronic medical records.
- Current CPR certification if required by the state.
- Current driver's license and vehicle insurance, with access to dependable transportation.

Responsibilities
- Respond to clinical referral information, inquiries, and requests with care and professionalism.
- Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
- Provide direct nursing care and support to patients, prioritizing comfort and quality of life.
- Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
- Educate and advocate for patients and families, encouraging their participation in personalized care planning.
- Share responsibility for call duties.
