About The Position

At St. Croix Hospice, Registered Nurse Case Managers provide compassionate, individualized, holistic care to hospice patients in their homes. The role collaborates with the care team to develop and update personalized plans of care, leads end-of-life conversations, and supports effective symptom management with empathy and clinical excellence. The position offers meaningful patient connections and autonomy in nursing practice.

This is a full-time position based in Baraboo, Wisconsin, with a schedule of 8:00 a.m. to 5:00 p.m. and on-call expectations. Travel is listed as 100%.

Requirements

• Associate, bachelor’s, or master’s degree from an accredited nursing program (RN).
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver’s license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Collaborate with the care team to provide patient care and support end-of-life conversations and symptom management.
