Blended Case Manager
About The Position
RHA Health Services is hiring a Blended Case Manager to support people with intellectual and developmental disabilities and people facing mental health and substance use challenges. RHA’s mission is to provide a safe and healthy environment while creating opportunities for personal outcomes. The people it serves and supports have been at the center of its work for over 30 years, and the organization provides services in North Carolina, Georgia, Pennsylvania, Tennessee, and New Jersey. Pre-employment screening may include a criminal background check; checks of the OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities registry; drug testing; education verification and other credentialing based on position requirements; and employment history or references if required. For positions requiring driving, proof of a driver’s license, insurance, and vehicle may be required if transportation is part of the role.
