About The Position
Amedisys is hiring an RN case manager to provide hospice care to patients at home. The role involves assessing patients, collaborating with the care team to create and implement individualized plans of care, educating patients and caregivers, and maintaining accurate patient records. The position also includes assigned on-call responsibilities. Pay and shift details are placeholders in the posting.
Requirements
- One year of nursing experience.
- Current RN license specific to the state(s) where assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records accurately, promptly, and according to care center policies.
- Record changes and outcomes related to pain and symptom management, as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Communicate regularly with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
