About The Position

Optimal Case Management is hiring a full-time Coordinator of Community Services (CCS) to partner with individuals with intellectual and developmental disabilities and their families. The CCS builds relationships, learns what matters most to each person, helps coordinate services and resources, advocates for individual needs, and supports progress toward meaningful life goals. Positions serve Salisbury, Princess Anne, or Snow Hill, with applicants encouraged to apply for the region closest to home. This is a community-based role, not a traditional remote job. Administrative work may be completed from a home-based workspace, but regular in-person community work and regional travel are essential. Visits and meetings may take place in homes, workplaces, provider locations, programs, and other community settings. Travel depends on caseload needs and may occasionally involve driving up to approximately two hours one way. Reliable transportation and the ability to travel throughout the assigned region are required. The CCS independently manages a caseload, schedule, community visits, service coordination, follow-up, and documentation while collaborating with supervisors, other case managers, Quality Assurance, Training, providers, families, community partners, and organizational leadership. The role requires accurate and timely documentation, defined productivity expectations, confidentiality, organization, accountability, and follow-through. Travel between work locations is part of the workday but is not itself a billable case management service. Work is performed in a combination of home-based and community settings. Community environments may occasionally be unfamiliar or uncomfortable, or present potential health or safety concerns. Personal protective equipment may be required depending on the environment; employees are expected to use professionalism, situational awareness, sound judgment, and appropriate safety practices while working independently. The position offers a salary of $58,240–$60,320 annually. Benefits include medical, dental, and vision insurance; company-paid life and long-term disability insurance; company-paid holidays; paid time off; a 401(k) retirement plan with employer match; a supplemental benefits program; an employee assistance program; mileage reimbursement at the federal mileage rate; and professional development opportunities. Employment is contingent upon successful completion of applicable job-related pre-employment requirements. Applicants must not be employed by or provide services for more than one Maryland Medicaid provider at the same time. Optimal Case Management provides equal employment opportunities and reasonable accommodations in accordance with applicable law.
