About The Position

Optimal Case Management’s Coordinators of Community Services (CCS) partner with individuals with intellectual and developmental disabilities and their families to understand what matters most to them, navigate services and resources, advocate for their needs, and support progress toward meaningful life goals. This is relationship-centered work that requires independence, organization, accountability, travel, timely documentation, and consistent follow-through. The position is community-based, not a traditional remote job. Administrative work may be completed from a home-based workspace, but regular in-person community work is essential. CCS employees travel throughout an assigned service region and may meet individuals in their homes, provider locations, workplaces, programs, and other community settings. Travel varies with caseload needs and may occasionally involve driving up to approximately two hours one way. Reliable transportation and the ability to travel throughout the assigned region are required. CCS employees independently manage caseloads, schedules, documentation, and community visits while collaborating with supervisors, other Case Managers, Quality Assurance, Training, providers, families, community partners, and organizational leadership. The role includes defined productivity expectations and requires accurate, timely, person-centered documentation that reflects activities, needs or goals, how services relate to those goals, and appropriate next steps. Travel between work locations is part of the workday but is not itself a billable case management service. Work is performed in a combination of home-based and community settings, with regular travel throughout assigned counties and the service region. Community settings may be unfamiliar, uncomfortable, or present potential health or safety concerns. Appropriate personal protective equipment may be required depending on the environment. Employees are expected to use professionalism, situational awareness, sound judgment, and appropriate safety practices while working independently in the community.
