Optimal Case Management is hiring a full-time Coordinator of Community Services (CCS) to partner with individuals with intellectual and developmental disabilities and their families. The CCS listens to what matters most to each person, helps navigate services and resources, advocates for their needs, and supports progress toward meaningful life goals. The role combines relationship-centered case management with independence, organization, accountability, travel, timely documentation, and follow-through. This is a community-based position, not a traditional remote job. Administrative work may be completed from a home-based workspace, but regular in-person work throughout an assigned service region is essential. Travel varies with caseload needs and may occasionally involve driving up to approximately two hours one way. Meetings may take place in individuals’ homes, provider locations, workplaces, programs, and other community settings. Reliable transportation and the ability to travel throughout the assigned region are required. CCSs independently manage caseloads, schedules, community visits, coordination, and documentation, while collaborating with supervisors, other case managers, quality assurance and training teams, providers, families, community partners, and organizational leadership. Success requires balancing engagement, meetings, coordination, follow-up, documentation, travel, and administrative responsibilities. Employees must meet defined productivity expectations and document case-management activities accurately and within required timelines. Travel between work locations is part of the workday but is not itself a billable case-management service. The position requires a bachelor’s or graduate degree in social work or another health or human services field, plus qualifying supervised field experience or qualifying direct clinical-care experience. Preferred experience includes coordinating services for people in Medicaid or waiver programs and for people with intellectual or developmental disabilities, as well as experience with intake and referral, service monitoring, allied service-delivery systems, and applicable regulations. Work is performed in home-based and community settings, with regular travel throughout assigned counties and the service region. Some settings may be unfamiliar, uncomfortable, or present potential health or safety concerns. Appropriate personal protective equipment may be required. Employees are expected to use professionalism, situational awareness, sound judgment, and appropriate safety practices while working independently in the community. Employment is contingent on successful completion of applicable job-related pre-employment requirements. Applicants must not be employed by or provide services for more than one Maryland Medicaid provider at the same time.
