Optimal Case Management’s Coordinators of Community Services (CCS) partner with individuals with intellectual and developmental disabilities and their families to understand what matters most to them, navigate services and resources, advocate for their needs, and support progress toward meaningful life goals. The work is relationship-centered and person-centered, and requires independence, organization, accountability, travel, timely documentation, and consistent follow-through. This is a community-based position, not a traditional remote job. Administrative work may be completed from a home-based workspace, while regular in-person work throughout an assigned service region is essential. Meetings may take place in homes, provider locations, workplaces, programs, and other community settings. Travel varies with caseload needs and may occasionally involve driving up to approximately two hours one way. Reliable transportation and the ability to travel throughout the assigned region are required. CCS employees manage an assigned caseload and balance engagement with individuals and families, meetings, communication, community visits, service coordination and follow-up, documentation, travel, and administrative responsibilities. Accurate, timely, person-centered documentation must meet organizational and regulatory requirements and defined productivity expectations. Travel between work locations is part of the workday but is not itself a billable case management service. Employees work independently managing caseloads, schedules, documentation, and community visits, while collaborating with supervisors, other Case Managers, Quality Assurance, Training, providers, families, community partners, and organizational leadership. Work takes place in a combination of home-based and community settings. Community environments may occasionally be unfamiliar, uncomfortable, or present potential health or safety concerns. Appropriate personal protective equipment may be required depending on the environment. Professionalism, situational awareness, sound judgment, and appropriate safety practices are expected. The position is full-time, with an annual salary of $58,240–$60,320. Employment is contingent upon successful completion of applicable job-related pre-employment requirements. Applicants must not be employed by or provide services for more than one Maryland Medicaid provider at the same time.
