About The Position

The Family and Children’s Behavioral Heath Case Manager promotes PATH’s mission of helping individuals achieve a more independent and fulfilling life. As part of a team with a therapist, the Family Based Mental Health Worker provides intensive family-based services, primarily in the family’s home. Services include assessments, service coordination and linkages, crisis intervention and prevention, educational and daily living skills, collaboration with systems involved with the youth and family, and assistance with individual, family, and group therapy.

Requirements

Bachelor’s degree in social work, counseling, marriage and family, or psychology from an accredited college or university.
One year of experience working with children and families.
Valid driver’s license and access to a vehicle.
Good interpersonal skills.

Responsibilities

Provide initial and ongoing comprehensive assessments of identified families and youth.
Assist the therapist in providing individual, family, and group therapy to identified families.
Collaborate regularly with all systems involved with the youth and family.
Develop and implement socialization and daily living skills training and activities for the youth and family.
Provide case management services and linkages to needed services for the youth and family.
