SMI Case Manager

About The Position

Riverwood Center is seeking an SMI Case Manager to provide community-based case management services to individuals with severe mental illness and their families. The role helps consumers access health services, financial assistance, employment, education, social services, and natural supports through planning, linkage, advocacy, coordination, and monitoring. The Case Manager assesses needs, develops and monitors Person Centered Plans, and documents treatment accurately and promptly. The position may also coordinate care between the criminal justice system and consumers participating in the Berrien County Mental Health Court and/or prisoner re-entry initiative.

Requirements

Bachelor’s degree in an appropriate mental health discipline.
One or more years of experience providing treatment to individuals with mental illness.
Excellent computer and interpersonal skills.
Punctuality and the ability to organize and manage several projects or tasks simultaneously and work independently.
Reliable means of transportation for community outings.

Responsibilities

Provide case management services in the community to consumers with severe mental illness and their families.
