About The Position

Amedisys is hiring an RN Case Manager Hospice to provide hospice care to patients at home. The role performs patient assessments and collaborates with the care team to develop and implement individualized plans of care. The case manager also provides patient and caregiver education, monitors and documents care progress and symptom management, and communicates regularly with the clinical manager and care team. On-call responsibilities and services are performed as assigned.

Compensation may vary by geographic market and may depend on job-related knowledge, skills, and experience. Benefit eligibility may vary by position and shift status; participation in some benefits requires enrollment in an Amedisys medical plan.

Requirements

• At least one year of nursing experience.
• Current RN license specific to the state or states assigned for work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain accurate, timely, individualized patient care progress records according to care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Perform other duties as assigned.
