Requirements

• Graduate of an accredited school of nursing.
• Must have a current RN license to practice in the state of Indiana.
• Must hold current CPR certification or obtain within the orientation period.
• Must hold current TB certification or obtain within the orientation period.
• Must be able to provide those services requiring substantial specialized nursing skill.
• Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
• Must be able to drive independently to all patient/family clients’ homes.
• Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of him/her, or to others in the workplace.
• Must be comfortable working with hospice patients, with the emphasis on palliative care.
• Ability to recognize needs and emergencies of hospice and palliative care patients, and provide appropriate interventions.
• Strong interpersonal skills.
• Strong assessment skills.
• Must have visual and hearing ability to comprehend written and verbal communication.
• Must be able to engage in clear, concise verbal/nonverbal communication.
• Must have the ability to remain calm in occasional emergency situations.
• Recognizes emergencies and has knowledge of emergency procedures, according to Agency policy.
• Must be willing to travel between patients in inclement weather.
• Must be willing to accept assignments without regard to location, socioeconomic environment, disease, or ethnic origin.
• Must exhibit dependability in areas such as attendance, tardiness, and timely performance of duties.
• Must be willing to make scheduled visits on holidays, weekends, or evenings when assigned.

Nice To Haves

• Bachelor’s degree in Nursing preferred.
• Infusion skills preferred
• Prefer one (1) year of recent hospital or home health care experience.

Responsibilities

• Performs nursing procedures and care to assigned terminally ill patients by assessing needs and implementing nursing care plans under the coordination of the RN Case Manager.
• Implements Agency policies in all work-related situations; orientation of patient/family to hospice benefit services, including identifying the role of team members.
• Follow state and agency policy and procedures when legally pronouncing a death. Specifics of the death event will be documented in the clinical record; required information is communicated/submitted to appropriate parties. The patient’s body will be handled with respect and dignity and in accordance with individual patient/family requests.
• Respect the patient's or family's cultural and religious traditions and beliefs.
• Recognize and maintain professional boundaries in relationships with the patient/family/caregiver.
• Documents findings accurately, in a timely manner, and on appropriate forms, in accordance with the Agency's procedures.
• Delivers nursing care to patients, which includes pain/symptom management, teaching, providing direct care, case management, referral, advocacy, and monitoring.
• Supervises clinically the care delegated to LPNs, Home Health Aides, CNAs, Companions, and/or Homemakers.
• Instructs LPNs, Home Health Aides, and/or CNAs on specific patient care procedures within approved policies.
• Maintains a therapeutic relationship with patients/families. This includes the confidentiality of personal matters, patients', coworkers', and the Agency's business.
• Follows orders from the patient’s physician and informs the physician and other personnel of changes in the patient’s condition and needs.
• Demonstrates understanding of hospice philosophy and concept of care and considers the patient/family as a unit of care.
• Assists with the preparation, implementation, and continuing evaluation of the Interdisciplinary Plan of Care.
• Provides for the emotional and physical comfort and safety of patients. Adheres to Agency policies in all work-related situations.
• Observes, records, and reports to the appropriate person symptoms, reactions, and changes, including: General physical and mental condition of patients, and signs and symptoms that may indicate adverse changes; Stresses in human relationships between patients, between patients and personnel, and between patients and their families and visitors.
• Orients patient/family to hospice services, including identifying the role of the team member.
• Anticipates issues related to the patient's impending death and pursues resolution.
• Remains flexible for patient coverage and accepts responsibility under adverse conditions, such as weather-related crises.
• Maintains expected and acceptable productivity.
• Works in a cooperative relationship with other Agency personnel.
• Seeks appropriate clinical supervision.
• Participates in the Agency's committees and task forces.
• Assumes responsibility for continued personal growth and development.
• Participates in on-call rotation as assigned.
• Seeks educational opportunities to improve clinical practice and participates in in-service programs.
• Adheres to all the Agency's policies.
• Any other duties as assigned by the supervisor.
• Adheres to Stillwater Hospice privacy practices as described in the Notice of Privacy Practices.
• Participates in quality improvement activities and/or other agency committees or task forces when requested.
• Proficient and timely in all aspects of clinical documentation, including computer data entry.
• Proficient in blood draws, all routine patient treatments (dressing changes, catheters, etc.), pain and symptom management, care of the patient and family, including pediatric and HIV + patients; Mantoux administration.
• Proficient in all aspects of IV care, including programming of IV pumps, tubing changes, site care, and subcutaneous access.
• Display compassion, be a team player, have an optimistic outlook, be adaptive, accountable, and customer-driven.
• Provides personal care.
• Participates in QA by reviewing patient records to ensure compliance.
