If you're an RN who wants your work to mean something beyond a chart, this is your role. As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most.

Requirements

  • Active RN license in the state of employment
  • Graduate of an accredited school of Nursing.
  • Compassion for patients and families navigating end-of-life care
  • Strong clinical judgment and the ability to work independently in the field
  • Excellent communication skills across care teams and families

Nice To Haves

  • One (1) + year Hospice, home health or related organization experience preferred.

Responsibilities

  • Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
  • Building and adjusting personalized care plans in collaboration with physicians and care teams
  • Administering medications and treatments per physician orders
  • Keeping clinical documentation accurate, timely, and meaningful
  • Serving as the communication bridge between families, physicians, and community health partners
  • Educating patients and caregivers so they feel confident and supported
  • Participating in on-call rotation per team policy
