About The Position

Requirements

• Associate's degree in nursing
• Currently licensed by the state of Alabama
• Minimal 2 years' experience in a hospital setting working in medsurg, critical care or ER
• Must exhibit strong critical thinking and organizational skills
• Must be able to work independently and make independent decisions
• Must be flexible in scheduling
• Must be able to organize and prioritize a schedule to meet patient needs

Nice To Haves

• Previous home care or hospice experience

Responsibilities

• Evaluate and admit patients to home care
• Develop plan of care and provide care to patient according to plan
• Act as case manager making necessary adjustments in the patient's plan of care related to symptom changes and changes in care needs
• Review and plan patient care and or discharge during scheduled case conference meeting and document accordingly
• Act as liaison for patient/caregiver with MD and community healthcare partners
• Strive to lower cost of care through managing mileage, supplies and visit scheduling
• Completes all documentation related to patient contact, assessments, SOC, and recerts
• Resumption of care and discharge according to policy
• Maintain understanding of current home care regulatory guidelines
• Provide on-call coverage for both home care and hospice as assigned
• Demonstrate and encourage team behavior and exceptional patient/guest experiences
• Uphold and promote patient safety and quality
