About The Position
Serve as a patient ally throughout the continuum of care. Coordinate the full breadth of the patient experience, from E.R. to discharge. Then, go a step beyond by identifying patients with symptoms across recurring episodes and direct them to the specialist or service they need. Tap into community resources, like neighborhood groups, and up the ante on patient education for health issues seen throughout New York.
Requirements
- Bachelor's degree
- Fluent in Spanish
- Minimum of 2 years experience in primary or community health
- Knowledge of overall hospital and community resources
