About The Position

Requirements

• Current and unrestricted occupational therapy licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• At least six months of occupational therapy experience.

Responsibilities

• Assess and screen patients’ daily living and work-related skills, and develop therapeutic retraining programs with measurable objectives.
• Administer and interpret diagnostic and prognostic tests of function in patients’ homes or other settings to help physicians evaluate patients’ functional levels.
• Confer with patients’ physicians and other health care team members and participate in developing and revising plans of care and treatment.
• Provide therapeutic treatment and instruction in accordance with physician orders to improve or restore strength, coordination, range of motion, and function, or teach compensatory measures.
• Review and expand retraining programs in a manner consistent with each patient’s behavioral goals and the overall rehabilitation program.
• Consult with other vocational team members, as appropriate, to develop and implement vocationally oriented plans that meet patients’ needs and capabilities.
• Report patient status and progress to physicians, rehabilitation staff, the Clinical Manager, patients, and family members.
• Participate in care coordination and discharge planning, and serve as a resource to other health care personnel in meeting patient needs.
• Design community reintegration activities to support physical reconditioning and/or psychological adjustment, coordinating plans with the interdisciplinary team.
• Recommend or design special adaptive equipment to improve clients’ residential or work environments or participation in rehabilitation and community activities.
• Design and train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, and participate in team and family education meetings.
• Supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
• Accurately and thoroughly document patients’ care observations, interventions, and evaluations on the day services are rendered.
• Ensure interim verbal orders from physicians are accurately documented and implemented.
• Submit evaluations, treatment plans, and discharge summaries to the supervisor and care management staff.
