About The Position

The Occupational Therapist will provide long-term, relationship-based care to elderly participants in a community-based PACE program. The role supports participants in maintaining independence in their homes and communities through adaptive strategies, home modifications, cognitive interventions, and caregiver education. Unlike acute-care settings, PACE enables ongoing therapeutic relationships and the opportunity to observe the long-term impact of clinical care. The Occupational Therapist provides evaluations and ongoing treatment focused on activities of daily living, fall prevention, home safety, cognitive maintenance, adaptive equipment, and caregiver education. The role collaborates with an interdisciplinary team within a holistic, person-centered care model.
