About The Position

This is a full-time Registered Nurse position located in Shawnee, OK. The role involves providing comprehensive care management for assigned patients, including supervising nursing staff, home health aides, social work, and therapies. The nurse will be responsible for conducting head-to-toe assessments, administering medications, and completing patient care assessments and treatments as ordered by a physician. A key aspect of the role is coordinating work schedules to meet patient needs and ensuring timely electronic documentation of all patient visits within 24 hours. The position also requires developing a Plan of Care and communicating with the patient's physician, as well as making PRN visits as needed.

Requirements

  • Current Registered Nursing license approved by the Oklahoma Nursing Board of Regents without restrictions.
  • At least one year experience in either home health setting or acute-care setting with experience in elderly, middle aged, and/or pediatric care.
  • Must be able to lift a minimum of 20 lbs without difficulty.
  • Must own a vehicle in which to make home visits and maintain current automobile insurance coverage.
  • Must have a telephone available for after-hours calls when needed.

Responsibilities

  • Supervise and manage all care given to assigned case management patients.
  • Identify patient and/or family needs and oversee all patient care management for assigned patients, to include nursing staff, home health aides, social work and all therapies that are assigned to patients as ordered by physician.
  • Provide supportive care and appropriate services in accordance with attending physician's orders and the interdisciplinary team plan of care.
  • Coordinate work schedule to fit patient's time and needs.
  • Spend minimum 30 minutes to 1 hour in length in each home to do total head-to-toe assessments, vital signs, administer medications as ordered, complete supervisory of patient care assessments and give treatments as ordered by physician.
  • Complete and submit all patient visit documentation electronically no later than 24 hours of the time patients visits are completed.
  • Develop Plan of Care with all orders and communication going to patient's physician.
  • Make PRN visits as they occur and out of necessity.
